Rehabilitation for the elderly

Are you admitted to a hospital for a specialized medical treatment? For example, as a result of a stroke, bone fracture, or for a new knee or hip. After this, you cannot go directly back home because you need care. Then you need Geriatric Rehabilitation Care.

Medifit HCB provides this GRC at the nursing home in Teulada. We prepare you so that you can safely return home.

Stay and treatment at the Medifit Teulada nursing home

During the geriatric rehabilitation program, you stay on a full-board basis at the Ana Maria Espinós Llopis nursing home located in Teulada. The rehabilitation process is provided by Medifit HCB. The treatments are carried out in our treatment rooms, exercise areas, and outdoors.

The admission usually occurs immediately after hospitalization or if an acute condition arises, resulting in acute mobility disorders or a decrease in self-reliance.

The duration of geriatric rehabilitation is typically 6 to 8 weeks on average.

Step 1

Identifying the causes

  • History (what happened), education/information about the condition conducted by a specialist.
  • Physical examination;
  • Psychological examination;
  • Nutrition consultation.

Step 2

The creation of a customized treatment program

With the data from these examinations, a consultation with all medical stakeholders will create the program. The various treatments, such as cognitive physiotherapy, manual physiotherapy, medical training therapy, psychotherapy, nutrition counseling, and relaxation therapy, are coordinated and constantly adjusted through retesting and interdisciplinary consultation.

Geriatric rehabilitation care based on scientific research

The intensity of the rehabilitation is determined based on the intake and examinations. We exercise at the level of the patient. The exercises are aimed at achieving a safe and as swift as possible return home. We use the most recent scientific research for our treatments.

Duration of the program

The duration of the program is typically 2 to 6 weeks on average. The maximum duration of the program extends up to 6 months. It depends on the condition and the rate of progress. Before you are admitted to us, we review all reports. Upon arrival, various examinations are conducted to assess your symptoms. From here, we develop a tailor-made treatment program together with you. This geriatric rehabilitation program aims to get you back home as quickly and safely as possible. We also provide our care upon your return home if necessary.

Reimbursement for Geriatric Rehabilitation Care (GRC)

Geriatric Rehabilitation Care is intended for vulnerable elderly individuals with 2 or more conditions who require care from various disciplines before returning home. Geriatric Rehabilitation Care falls under the responsibility of a geriatrician/rehabilitation doctor. It’s not the case that older people always qualify for geriatric rehabilitation after a hospitalization. There are also other arrangements for temporary stays in an institution.
